Cervical Spine Surgery Overview

Cervical spine (neck) is a well-organised structure and composed of 7 bones called cervical vertebrae which are linked by nerves, muscles, ligaments and tendons. Cervical spine houses the spinal cord that sends messages from the brain to control all aspects of the body and also allows neck movement in all directions.

Cervical spine surgery is performed to relieve neck pain, numbness, tingling and weakness, to restore nerve function and prevent abnormal motion in your neck. It is done as a decompression surgery to treat nerve/spinal cord impingement or a fusion surgery to treat spinal instability. Many degenerative diseases can affect the normal curvature of spine and are treated through cervical spine surgery. Cervical spine surgery can be done either through front (anterior) approach or back (posterior) approach or a combination of both ways.

PREPARING FOR THE SURGERY

To make sure you are a candidate for cervical spine surgery, your doctor will perform detailed physical examination, neurological examination of your spine and neck area to check for alignments, curvature, range of motions, cause of pain and other nerve changes in your reflexes and muscle strength. Various diagnostic tests are also done such as; X-ray, MRI, bone scan and Myelogram.

You will be instructed to stop any over-the-counter pain relieving medicines before the surgery. If you smoke, you will need to stop smoking 2 weeks before the surgery.

ON THE DAY OF THE SURGERY

Before shifting to the operation theatre you will be given antibiotics to prevent the occurrence of infection after the surgery. Cervical spine surgery is done under general anaesthesia means you will be asleep during the procedure. Anterior cervical discectomy or decompression is the most common procedure done for spinal cord degeneration and severe neck pain.

During the surgery involving most common front (anterior) approach, one or two inches long skin incision are made on left/right side of the neck depending upon which part needs to be repaired. The affected disc is completely removed including any impinging disc fragments or osteophytes (bone spurs). The disc space is distracted to its prior normal disc height to help decompress (relieve pressure) on the nerves. Fluoroscopy is done to get X-ray of the spine during the surgery to confirm the correct disc level of the spine. After confirming, the exact disc is removed and an artificial disc device is implanted in the cervical disc space. Incision is closed and fixed with staples and the area is covered with bandage.

POST-SURGERY

It takes about two hours for surgery to complete, after which you will be kept in the recovery room where your vital signs like heart rate, BP, respiratory rate, temperature are monitored. Once the effect of anaesthesia gets over, your vital signs become stable and you are alert, you will be shifted to a normal hospital room. You may require hospital stay of one to two days.

You will be wearing a neck collar/brace after the surgery and physiotherapist will plan your exercise program which includes leg exercises, walking, stair climbing and so on. Physiotherapist will help you to get back to your normal routine and by the time of discharge you would be able to get in and back of the bed by yourself, walk without use of walker or climb few stairs.

RECOVERY-PROCESS

Before you leave hospital you will be given instructions about how to use collar brace and an exercise program as recovery after cervical spine surgery highly depends on physical exercise and after-care. Since, walking is the best way to heal tissues, there are no restrictions for walking or stair climbing. You should not lift any objects heavier than 5 kg, especially over your head, for the first four to six weeks at home. You may feel pain or soreness at the site of graft which can be managed by ice or warm compresses.  Do not use lotions or oil on your incision while it is in the healing process and if skin irritation occurs try wearing a silk scarf under your brace to help decrease irritation.

Recovery time may vary in different individuals, usually people can start doing light work after a week of surgery.
